They are looking for two highly motivated and committed First Contact Stroke Support Workers to join the new team in Brighton to support stroke survivors and carers and ensure they get the right care at the right time.

HERE is running a pilot Stroke Catalyst project supporting the implementation of the Integrated Community Stroke Service in Brighton & Hove in partnership with NHS Sussex, Sussex Integrated Stroke Delivery Network and the voluntary sector. The project aims to transform stroke services and enable new ways of supporting stroke survivors and carers.

You will be at the forefront of supporting HERE to deliver a new stroke project, they encourage applications from anyone with an interest in improving healthcare services and delivering personalised care. They welcome applications from those looking for secondment opportunities.
