Crawley Social Prescribing

 

Welcome to Crawley Social Prescribing, a service from Crawley Community Action and Crawley Clinical Commissioning Group.

Our social prescribing Link Workers collaborate closely with PCN hosted link workers to ensure that social prescribing reaches as many people in need as possible across Crawley.

We partner with local organisations and our link workers can identify and address gaps in the community as well as supporting the sustainability of groups.

Social prescribing links patients with non-medical support to improve their physical and mental wellbeing. It can help people with nutrition, housing, employment issue, and much, much more.

Who can we help?

Social prescribing works for a wide range of people, including those:

  • who are 18+
  • with one or more long-term conditions
  • who need support with anxiety or depression
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing.
