Crawley Social Prescribing

Welcome to Crawley Social Prescribing, a service from Crawley Community Action and Crawley Clinical Commissioning Group.

Our team of five Link Workers collaborate closely with PCN hosted link workers to ensure that social prescribing reaches as many people in need as possible across Crawley.

Referrals to Crawley Social Prescribing are made by GPs and staff from practices across Crawley.

What is social prescribing? 

Social prescribing is all about linking patients with non-medical support to improve their physical and mental wellbeing. It can help people with their nutrition, their housing or their employment issue, and much, much more.

Link workers support people by understanding what matters to them and helping them connect to community groups and statutory services. We put people at the centre of what we do and work together to find the best solutions for them.

Who can we help?

Social prescribing works for a wide range of people, including those:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing.

Working with the local community

Crawley Social Prescribing connects to organisations that have signed up to our Guarantee. Our Link Workers make referrals to selected services from those organisations, which are included our social prescribing database, Simply Connect Crawley.
