Crawley Social Prescribing supports people by understanding what matters to them and helping them to connect to local services and community groups. We put people at the centre of what we do and work with them to find the best solutions for their needs.

Crawley Social Prescribing offers support for your non-medical needs, connecting you with the people or advice that matters to you. The service is free and confidential; below are just some of the areas in which we can offer support:

How to link up to a social prescriber

Patients registered with Southgate Medical Group can now self-refer to our Social Prescribing service.

Linking up with one of our team is easy. You can:

  • call us on 07719 103290.
  • complete your details below and click ‘Submit’.

A social prescribing link worker will contact you within 5 days to talk about your needs and make sure that this is the right service for you.

  1. Patients registered with all other surgeries in Crawley

All staff from your GP surgery can refer you for social prescribing. Call your surgery and ask to be referred to a social prescriber.
