Crawley Social Prescribing works with:

  • GP practices across Crawley

Our team of five Link Workers collaborate closely with PCN hosted link workers. Patients can be referred to Crawley Social Prescribing by GPs and staff from any of the surgeries.

  • local community groups and statutory services

We work with local partners to make referrals and signpost.

Working with the local community

Our Link Workers make referrals to selected services from partner organisations which are included on our social prescribing database, Simply Connect Crawley.
