AUKWSBH has an exciting opportunity to join the Prevention Assessment Team and make a real difference to the people of West Sussex. They are seeking to fill two Community Support Worker vacancies covering West Sussex.
Are you super organised and enjoy working with people? If you are competent and confident with IT and working with clients over the phone and face to face, this could be the role for you.
AUKWSBH are contracted to deliver the third sector arm of the WSCC Prevention Assessment Team (PAT). PAT is a multi-agency, multidisciplinary team delivering a preventative service across West Sussex. The team includes NHS health advisors (qualified health professionals), WSCC social care workers and AUKWSBH community support workers who oversee and triage referrals into the team, provide advice and support over the telephone or see clients in their own home. The team’s objective is to improve quality of life and the promotion of health and wellbeing.
The roles will primarily be to support the triage element of the service provided by AUKWSBH but will also develop to manage a small caseload providing low level advice and guidance to individuals.
