The Role:
Due to the expansion of Social Prescribing Service, they have some exciting opportunities for new Social Prescribers to join the amazing and dedicated team in various locations across West Sussex. You will be a key member of the Primary Care Network (PCN) multi-disciplinary team and will provide support to patients, empowering them to take control of their health and well-being.
Although AUKWSBH supports people over the age of 50, the Social Prescribers support all patients from the age of 18.
These posts are exciting opportunities, giving the successful candidates the opportunity to deliver a service which will improve people’s lives and make a real difference. All roles offer hybrid working, with a mixture of working in GP Surgeries, office and homeworking.
Bognor Regis – You will need to cover Monday and Friday and other days are flexible and can be discussed at interview.
Brighton – working days can be discussed at interview.
Arun and Chanctonbury – both full time roles but will offer flexible working patterns which can be discussed at interview.
