Salary: £8,716 pa (£21,500 FTE)

They are looking for a Coordinator to join the amazing Crisis Team. Ideally, they are looking for an experienced Crisis Care Coordinator, but would consider someone willing to learn the role for the right candidate.

  • The Crisis Team works across a 7 day week, 8am – 8pm, providing short-term, person centred care to people in later life, who maybe returning home from hospital or for those who find themselves in need.
  • As Care Coordinator, you will work closely with the manager and the other coordinators of the service, to ensure referrals are processed in a safe and timely way, ensuring the rota is covered and carers and clients are well supported.
  • You will be working as part of the Crisis team, ensuring safe and efficient delivery of care is met within the client’s homes.
  • The role will also ensure adequate levels of care are provided and support with covering calls in respect of any annual leave, training days and sickness.
  • They are looking to recruit enthusiastic individuals who enjoy working with people to enable them to adopt an independent lifestyle.
  • You will need to be organised with good administration, observation and communication skills.
  • They would consider job share for the right candidates.
  • A driving license is essential to meet the needs of the service.
