Working hours : 37 hours a week and 30 hours a week (2 roles)

Salary: £21,500 FTE and £17,432 pa pro rata for 30 hours
Location: Horsham, County hall

 

 

Age UK West Sussex, Brighton and Hove are contracted to deliver the third sector arm of the WSCC Prevention Assessment Team. The Prevention Assessment Team (PAT) is a multi-agency, multidisciplinary team delivering a preventative service across West Sussex.

The team includes NHS health advisors (qualified health professionals), WSCC social care workers and AUKWSBH community support workers who oversee and triage referrals into the team and provide advice and support over the telephone or come and see clients in their own home. The team’s objective is to improve quality of life and the promotion of health and wellbeing.

  • The roles will primarily be to support the triage element of the service provided by AUKWSBH but will also develop to manage a small caseload providing low level advice and guidance to individuals.
  • Full drivers licence is required for this role.
