Confidentiality

What you share with your Social Prescriber / Link Worker will be treated as confidential.

Information will only be shared:

• when it helps support you
• with relevant support services (which may include your GP, if appropriate and agreed)
• and only what is necessary

Wherever possible, this will be discussed with you first.

 

When confidentiality may be shared without consent

There are rare situations where information may need to be shared without your agreement:

• if there is serious concern about your safety or someone else’s safety
• or if we are legally required to do so

Wherever possible, we would explain this and involve you.

 

How your information is used

The Social Prescribing service is funded by the NHS, so we are required to show that it is helping people. This usually involves sharing anonimised information for monitoring and evaluation.

Occasionally, we may also share anonimised case studies or examples of support provided to help commissioners understand the difference the service makes. These are written in a way that does not identify you.

Your name or identifying details are never included in reports.

Your information will be stored securely in line with data protection law. For more information, please see our Privacy Notice.

 

Follow-up contact

We may contact you in future to ask about the longer-term impact of the support you received, unless you would prefer us not to.

 

About the service

Social Prescribers / Link Workers are not medically trained and cannot give medical advice. If you have medical concerns, or are planning major lifestyle changes, please speak with your GP.
